To participate, please provide the following information on your company's letterhead. If you would like to place an order as soon as your account is set up, please note that on the letterhead and we will call you to take your order once your account is active.
On your organization's letterhead, please include:
- Company Name
- Contact Name(s) (allowed up to 3)
- Phone number(s)
- Email address(es)
Along with your letterhead, please include a copy of your business' W-9 tax identification form. Send your documents to us in one of three ways:
- Email both documents as a pdf to [email protected]
- Fax both documents to 717-960-1356 to Attn: Gift Card Department
- Mail both documents to: Gift Card Fulfillment, Ahold USA, P.O. Box 249 Carlisle, PA 17013
New account set-up takes 7 to 10 days to process. You will receive a letter in the mail, which will include your account number and ordering instructions. Orders take 5 to 7 business days from the date of order to arrive at the requested store location.
